Stages of Life Aesthetics – Terms and Conditions


Reservations

Stages of Life Aesthetics reservations are highly recommended. A valid credit card or gift card,
deposit is required. All major credit cards are accepted.

Check-In
Please plan to arrive at Stages of Life Aesthetics 15 minutes before your scheduled appointment
time.


Allow sufficient time for check-in, We aim to provide the best spa experience possible for you and
for the next patient.your treatment will end at the originally scheduled time regardless of your
arrival.
We encourage you to arrive in comfortable attire and to avoid bringing jewelry or other valuables.
Stages of Life Aesthetics is not responsible for the loss of personal belongings. Robes, slippers, and
beverages are provided for your comfort.
If a guest arrives late, the treatment will be shortened accordingly. If the delay exceeds 15 minutes,the service will be considered a no-show and charged in full.

Cancellations & Rescheduling

If you need to cancel or reschedule, please call, text or do it on line at least 24 hours in advance.
Failure to show up for your scheduled appointment will result in a 100% charge of the treatment cost to the card on file.

Age Limitations / Pre-Natal Treatments

All treatments are available to guests 18 years of age and older.
Guests under 18 must sing a minor consent form and have parental supervision during
treatments.
Pre-natal massages are available only after 12 weeks of pregnancy.

Membership Cancellations

Memberships are non-refundable.

Gift Cards

Gift Cards and Online Gift Certificates are non-transferable and non-refundable.
Gift Certificates must be presented before services are rendered; otherwise, an alternative form of payment will be required.


A valid ID is required at redemption.


Gift Cards cannot be used for product purchases, promotional offers, or gratuities unless otherwise specified.


When a Gift Certificate is purchased for a treatment package, the package must be completed in a single visit; otherwise, it will be honored at its face value.
Gift Cards are valid three (3) days after purchase.


Stages of Life Aesthetics is not responsible for lost, stolen, damaged, or unauthorized use of Gift
Cards or Certificates.
In-store Gift Cards are redeemable only at the location where they were purchased.

Health & Safety
Please inform us of any health conditions when booking your appointment, including high blood
pressure, heart conditions, allergies, or pregnancy.
This is a Med Spa, upon arrival, you may be asked to complete a comprehensive health history
form to help us tailor your treatment safely and effectively Thank you for your patience with this
process. It is only done once a year.

Introductory Offers

Introductory Offers are valid for first-time guests only and apply to select treatments.
These offers cannot be combined with other promotions or used with Gift Cards, Gift Certificates,
or third-party programs (e.g., Spa Finder, Spa Wish, or Spa Week).
Restrictions apply.

Gratuities

Gratuities are not included in service prices. A 20% gratuity based on the regular menu price is
suggested for your provider’s exceptional service. But not limited to.

Timing of Services

90-minute session: 80 minutes of hands-on service, 10 minutes for consultation and dressing.

60-minute session: 50 minutes of hands-on service, 10 minutes for consultation and dressing.

30-minute session: 25 minutes of hands-on service, 5 minutes for consultation and dressing.

Client Comfort and Medical Disclaimer
All services at Stages of Life Aesthetics are performed by license professional under Dr David Klein
Supervision, Filers and injectables are performance by Dr Ada Rivera. We have a variety of
regulated and non- regulated procedures and are designed for relaxation, rejuvenation, and
aesthetic well-being.

By receiving any service, you acknowledge that:
You have disclosed any medical conditions, allergies, injuries, or medications that could affect your
treatment.
You voluntarily assume all risks associated with the Med spa services and facility use, including
possible soreness, bruising, allergic reactions, or other minor effects.
You understand the inherent risk of exposure to communicable diseases, including COVID-19, while
visiting the premises.
Waiver and Release of Liability
By using the facilities and receiving treatments at Stages of Life Aesthetics, you agree to release,
indemnify, and hold harmless Stages of Life Aesthetics, its owners, employees, agents, affiliates,
and successors from any and all liability, claims, damages, or expenses arising out of your
participation in spa services or use of the facilities.

This Waiver and Release of Liability is governed by the laws of the State of Florida.
Any dispute shall be resolved exclusively in the Circuit Court of the Ninth Judicial Circuit in and for
Seminoles County, Florida, and both parties expressly waive the right to a jury trial.

If any provision of this Waiver is deemed invalid, the remainder shall continue in full force and
effect.

terms and conditions outlined above.
